THE FOLLOWING FORMS ARE REQUIRED BY ALL PARTICIPANTS:

• Entry Form
• Official Abstract Form (2 Copies) - ONLINE PRE-FILLABLE PDF
• Form 1: Sponsor Checklist & Safety Assessment
• Form 1A: Student Checklist
• Form 1B: Approval Form
• Form 1C: Registered Research Institutional / Industrial Setting Form

THE FOLLOWING FORMS ARE REQUIRED ONLY FOR SPECIFIC PROJECTS:

• Form 2: Qualified Scientist Form
• Form 3: Risk Assessment Form
• Form 4: Human Subjects and Informed Consent Form
• Form 5A: Vertebrate Animal Form
• Form 5B: Vertebrate Animal Form
• Form 6A: Potentially Hazardous Biological Agents Form
• Form 6B: Human and Vertebrate Animal Tissue Form
• Form 7: Continuation Projects Form

Georgia College & State University Regional Science & Engineering Fair Required Forms:

• Intent to Participate Form
  SUBMIT VIA WEB OR MAIL ON/BEFORE JANUARY 12, 2009
  
  
• Signed GCSU Registration Form and Checklist
  Submit with all required forms and $15 Application Fee by 5PM on JANUARY 19, 2009
  Note: Applications received after January 19, 2009 will not be processed.
